Exit Survey

As a former employee, we want to understand your thoughts about our facility and use them to make this a better place for employees to work. Your input will provide valuable feedback that will enable BMC to recognize areas of excellence; identify areas for improvement and monitor trends in quality over time.

We are asking your participation in an exit interview process by completing the Employee Exit Survey. Be assured that your responses are confidential. Your responses will go directly to the Human Resources Department for processing and analysis.

You are encouraged to provide your open and honest responses. You do not have to answer every question. We appreciate your input and want to learn from employees that leave the facility as to how we can improve our hospital.
